Download the 1PDX2026 Electronic Signature Sheet.

Step 2:

Print the Signature Sheet on at least 20 pound, 8.5" x 11" uncoated white paper, or equivalent.

Step 3:

Fill out, sign, and date the form. Be sure to include your complete street address, including zip code.
